Commissioned by Brighton & Hove City Council, we offer innovative and engaging tier 2 weight management programmes to support residents to lose weight in a way that works for them. 

How to refer

1. Your Clinical Operating System e.g. Systm One or EMIS Web

You can easily refer through your clinical operating system. The referral goes straight to the Brighton & Hove Healthy Lifestyles Team, and they will contact the client and triage.  

Submit a referral on behalf of the patient through Brighton & Hove’s healthy lifestyle form. The referral goes straight to the Brighton & Hove Healthy Lifestyles Team, and they will contact the client and triage.

All patients can self-refer through the ‘Sign Up’ button on this website. It’s quick, simple and lets patients pick the programme that’s right for them.

*Patients may still be referred if deemed appropriate by specialist staff, such as a consultant or specialist weight management nurse.

Raise the issue: empowering conversations with your patients

Discussing weight management and other lifestyle changes with a patient can be difficult. Achieve better patient outcomes by learning tried and tested ways to have positive conversations around preventing diabetes, weight management, obesity and smoking cessation.
